The DSM Group is a great tool for developing the health literacy of your patients with diabetes and reinforcing the valuable information they receive from your team.

    About the group:
  • DSM is co-facilitated by a specialty diabetes nurse, and a dietitian
  • Each course is designed around the needs of the group and could cover anything from managing medication through to eating well and being active
  • Patients learn directly from each other, gain support, and share their experiences
  • Topics are covered in a practical way that encourages participation

This course qualifies as one of the options for self-management for Kia Ora Vision.

Bookings are preferably by self-referral. We’ve found that self-referrals are best because it shows that person has already bought into the idea of self-management.

What the participants have said: “Thoroughly enjoyed the programme. What I learnt is that you aren’t the only one in the same boat.”

“Appreciated hearing some of the other people’s stories, realising everyone is on a different part of a similar journey.”

“Great understanding of what diabetes is and how we can control it better.”

The course runs for 2.5 hours per week for three weeks. Download the flyer here and the brochure here.
