What is Kia Ora Vision?
A new flexible programme which aims to improve the health of people with long-term health conditions such as:

  • Diabetes
  • Heart Disease
  • Lung Disease
  • Depression
  • Pain
  • And others

Is Kia Ora Vision for me?
If you have a long-term condition that you would like to understand more about, talk to your doctor or nurse.

What about CarePlus?
Kia ora Vision has replaced CarePlus, which mainly offered free appointments. Rather than free appointments, through Kia Ora Vision, your GP Practice Team (and other health and social providers in their network) will help you create your own plan and support you to achieve goals that matter to you.

What does Kia Ora Vision offer?
Your doctor or nurse will help you to set goals that matter to you. ‘What matters to you’ will be the basis of a plan that can be shared with whomever you wish, such as:

  • Your family/whanau
  • Community services you may be using (e.g. District Nursing or Physiotherapy)
  • Outpatient clinics and hospital specialists

Self-management is central to Kia Ora Vision. This means talking about your circumstances; thinking about your health and lifestyle and setting achievable goals. Your nurse can connect you with the right people who can help.
