Long term conditions continue to be the leading cause of hospitalisations in NZ with Maori over represented in these admissions.

This is a strong focus area for Manaia and with the recently established Long Term Conditions Team Leader position to lead the education and health partnership to people with long term conditions in our community, we are able to offer significant dedicated support to Primary Care providers.

The role will have oversight of our Kia Ora Vision general practice based programme, and provide clinical and professional leadership expertise and support to primary healthcare providers ensuring competent and confident management of those with long term conditions.

 Skills essential for the positon are:

- Registered Nurse/Nurse Specialist

- Minimum of 2 years or equivalent working in the practice area of long term conditions

- Recent experience working in primary healthcare

- Skills and experience in adult teaching, coaching and or mentoring

- Excellent and proven interpersonal and relationship building skills

- Proficient in written communication, time management, Microsoft Office, and other general practice systems (ie Medtech, Dr Info etc)

- Must be familiar with models of nursing including Maori models of health.
