Extended GP consults now available for Tiakina Te Kaitiaki (Caring for the Carer) service

Tiakina Te Kaitiaki
This service provides support for carers of people who have a life-limiting illness, who are likely in their last year of life. The key purpose of this service is to enhance carer resilience and wellbeing. Tiakina Te Kaitiaki was established to focus on better meeting the informational, emotional, and physical needs of the informal, unpaid, carers for those with life limiting illness in the last year of life. It is important that we focus upon the carers of patients with long term conditions (such as stroke, dementia, chronic renal, chronic respiratory) to affect better outcomes all round.

How to refer to Tiakina Te Kaitiaki
Referrals are made via Healthlink -> Care Select > Manaia PHO > Kaitiaki service.

Funded extended GP consults
A funded extended consultation is available now for carers engaged with Tiakina Te Kaitiaki. This provides an opportunity for the carer, the cared-for person, whānau, family, and other service providers to meet with the cared for person’s GP (and nurse). It is an opportunity to discuss issues, clarify goals and plan future care (including advanced care planning conversations). Please note that extended consults can only be claimed for patients who are engaged with the Tiakina Te Kaitiaki service. These patients can be referred prior to, or at the time of, the extended consult.

How to claim
A simple report must be completed and sent with the invoice to accounts@manaiapho.co.nz. The report template is available from Practice Managers.
