Are you looking to develop your career in a different direction while utilising your existing Nursing skills?  This opportunity to improve primary care rapid response, coordinate services and enable appropriate management of people in primary care might be the new challenge you are looking for. 

The purpose of the Primary Options Programme is to ensure better support, more timely and more convenient healthcare services, and involves coordination of services based on best practice evidence and clinical pathways to enable safe and appropriate management of people in the primary care sector thereby reducing hospital admissions.

The continuation of the Primary Options programme is essential to long term successful primary health outcomes in our community, and with the implementation of the Acute Demand Service in conjunction with the NDHB, we have an exciting opportunity for an experienced Clinical Nurse to coordinate this service.

This role will be required to implement and manage the Primary Options Acute Demand Management service including providing a rapid and well-coordinated response to health providers requiring access to the service, and ensuring on-going support for providers in managing patient care under this service.

 

This role requires a Clinical Nurse with:

 

- A current NZ Registration with the NZ Nursing Council

- A post-graduate health/nursing qualification or currently working towards.

- 5 + years clinical experience in a health setting both primary and secondary

- A demonstrable understanding of the Northland context to health care delivery

- A demonstrable knowledge of community based services and a commitment to primary care

- Experience in innovative problem solving and service implementation

- A high level of IT competency including MS suite.

-Must excel in verbal and written communications and relations

- Experience in practice management systems Medtech 32 and/or profile would be desirable.

 

You must be a NZ Citizen or have a current NZ work visa.
